・大腰筋の外側縁を出て腰方形筋の前面 (腎臓の後方) を走る。
・肋下神経とほぼ並行するように下行する。
・腸骨稜の少し上で腹横筋を貫き、腹横筋と内腹斜筋に挟まれて状態で外側皮枝と前皮枝に分岐する。

「 日本人体解剖学 (上巻) 」および「 船戸和弥のホームページ 」には、他の神経との連絡に関する解説は見られないが、「 Wikipedia 」では「communication」という項で以下のように解説している。
「 The iliohypogastric nerve communicates with the subcostal nerve and ilioinguinal nerves. 」
以下は「 日本人体解剖学 (上巻) 」の解説文となる。
「 運動性および知覚性、主に第1腰神経から起こり、腰神経叢からの最上枝である。腰方形筋の前を肋下神経（第12肋間神経）に平行してその下方で腹横筋と内腹斜筋との間を腸骨稜の上縁に沿って外側方に走り、浅鼡径輪の少し上方で皮下に現れる。」

「 日本人体解剖学 (上巻) 」では腸骨下腹神経の枝として以下の3つを挙げている。

「 日本人体解剖学 (上巻) 」の腸骨下腹神経の項の筋枝の解説は以下のような簡単なものになる。
「 腹横筋の一部に至る。」
ただし、同書の内腹斜筋の解説の「神経支配」の項では、支配する神経として腸骨下腹神経をその1つとしている。
また、「 Wikipedia 」では、「筋枝」としての枝の解説はないが、以下のような解説文が見られる。
「 The iliohypogastric nerve is a nerve that originates from the lumbar plexus that supplies sensation to skin over the lateral gluteal region and motor to the internal and transverse abdominal muscles. 」

When harvesting bone from the anterior iliac crest (AICBG), the lateral cutaneous branch of the Iliohypogastric nerve (L1) is most likely to be injured.
Area supplied by the lateral cutaneous branch can be seen in blue near the hip. 」
