腓骨の外果関節面（下端（外果）の内側にある）の後方にある凹みのこと。参考にした資料によって若干異なるが、いくつかの靭帯が付着する場所となっている。
以下が外果溝に付着する靭帯として横脛腓靭帯を上げているサイトの解説文になる。
「a large rough depression on the medial aspect of the lower end of the fibula just behind the articular facet for the talus giving attachment to the posterior talofibular and the transverse tibiofibular ligaments.」
