　頚動脈鞘とは

　　

　　

 

　頚動脈鞘が主に包むものとして以下の4つが挙げれれる。

　1 . 総頚動脈 ： common carotid artery　頭部への動脈の主幹となり、内頚/外頚動脈に分岐

　2 . 内頸静脈 ： internal jugular vein　脳の血液を心臓に戻す主幹となる静脈

　3 . 迷走神経 ： vegus nerve　混合性の第Ⅹ脳神経で、頚部、胸部、腹部の内臓に広く分布している。

　4 . 深頚リンパ節 ： deep cervical lymph node

　「 Wikipedia 」には以下のような解説文が見られる。

「 In the upper part, the carotid sheath also contains the glossopharyngeal nerve (IX), the accessory nerve (XI), and the hypoglossal nerve (XII), which pierce the fascia of the carotid sheath. 」

　以下は「 日本人体解剖学 (下巻) 」の解説文となる。

「 頚部の両側には、胸鎖乳突筋の内面と椎前葉の間を上方に走る総頚動脈とその上方の続きの内頚・外頚動脈、さらにこれらに伴走する内頚静脈と迷走神経などの太い血管・神経束などがある。これらを共通に包む強い線維性の膜からできる血管神経鞘を頚動脈鞘という。 」

　また、「 船戸和弥のホームページ 」では以下のように解説している。

「 頚部内臓の両側を上下行する総頚動脈（上方では内、外頚動脈）、内頚静脈および迷走神経は共同の筋膜の鞘で包まれる。この鞘は頚筋膜の気管前葉およびこの外に接する浅葉のほか、これらと椎前葉とを連ねる筋膜で構成される。交感神経幹はこの鞘の後にある。肩甲舌骨筋が収縮すると、その中間腱と結合している頚動脈鞘が緊張し、それによって内頚静脈を広げ、頭部静脈血の還流を助けるという。」

「 The carotid sheath is an anatomical term for the fibrous connective tissue that surrounds the vascular compartment of the neck. It is part of the deep cervical fascia of the neck, below the superficial cervical fascia meaning the subcutaneous adipose tissue immediately beneath the skin. The deep cervical fascia of the neck includes four parts:

• 　・ The investing layer (encloses the SCM and Trapezius)　　　　　　　　 ： 浅葉　※
• 　・ The carotid sheath (encloses the vascular region of the neck)　　　　： 頚動脈鞘
• 　・ The pretracheal fascia (encloses the visceral region of the neck)　　： 気管前葉

• 　※ Weblioでは「investing layer」を「被覆筋膜」と訳しているが、「 日本人体解剖学 (下巻) 」でいう「浅葉」のことだと思われる。

　The carotid sheath is located at the lateral boundary of the retropharyngeal space at the level of the oropharynx on each side of the neck and deep to the sternocleidomastoid muscle, extending from the base of the skull to the first rib and sternum.

　The four major structures contained in the carotid sheath are:

• 　・ the common carotid artery as well as the internal carotid artery (medial)
• 　・ internal jugular vein (lateral)
• 　・ the vagus nerve (CN X) (posterior)
• 　・ the deep cervical lymph nodes

　The carotid artery lies medial to the internal jugular vein, and the vagus nerve is situated posteriorly between the two vessels.
In the upper part, the carotid sheath also contains the glossopharyngeal nerve (IX), the accessory nerve (XI), and the hypoglossal nerve (XII), which pierce the fascia of the carotid sheath.
The ansa cervicalis is embedded in the anterior wall of sheath. It is formed by "descendens hypoglossi" (C1) and "descendens cervicalis" (C2-C3).

　The three major fascial layers in the neck contribute to the carotid sheath: the investing fascia, the pretracheal fascia, and the prevertebral fascia.」
